Media Accreditation Application
To apply for accreditation to the Commonwealth Heads of Government Meeting (CHOGM) 2002, please complete the following application form.

MANDATORY REQUIREMENTS AND CONDITIONS
|
- Each representative must fill in a separate application form.
- Application forms must be received by FRIDAY 1 FEBRUARY, 2002.
- All sections marked * must be completed to ensure the accreditation application is eligible.
- Two (2) passport-sized photographs (name printed in ballpoint pen on the reverse side) of each individual seeking accreditation must be forwarded to the Task Force OR e-mail a digital image in jpeg format (full name to be included in digital image file name) to helen.mathers@pmc.gov.au Digital is the preferred format for photos.
- All applications must be accompanied by a letter from the media organisation to be represented at CHOGM 2002. This must be on original company letterhead, signed by the Political Editor, News Editor, or Chief of Staff acknowledging your representation of the organisation. If applying online, this letter must be forwarded as soon as possible to the Task Force at the address below.
- Faxes will not be accepted as an official application for accreditation.

PLEASE NOTE
|
- Media accreditation is granted by the Commonwealth Secretariat in London. Australia's Department of the Prime Minister and Cabinet is coordinating applications from media in Australia, New Zealand and other South Pacific nations on behalf of the Secretariat.
- The Commonwealth Secretariat reserves the right to limit accreditation numbers per organisation.
- Application for media accreditation is free.
- Completion and submission of this application form does not guarantee accreditation.
- Written confirmation on the status of the application will be provided after the closing date.
- The Media Centre is a non-smoking venue.
